An initial investigation revealed a Honda CR-V was headed east on Sunset Road when it crossed into the westbound lane and hit a Toyota RAV4 that was headed west.
A 17-year-old Port Washington boy was the driver of the Honda. Flight for Life transported him to Froedtert Hospital with severe injuries.
A 43-year-old Port Washington man was the driver of the Toyota. Fire department personnel had to extricate the driver and his passenger, a 32-year-old Port Washington woman.
The Toyota driver was also flown to Froedtert Hospital with life-threatening injuries. An ambulance took the passenger to a Grafton hospital.
Deviating from the designated lane and speed appear to be factors that contributed to the crash, which remains under investigation.
A GoFundMe has been set up for two of the injured.
